Livrant sa première musique pour la série, Kota Suzuki a dirigé une équipe de composition qui comprenait Hideki Okugawa, Akihiko Narita, Seiko Kobuchi et Wataru Hokoyama. Pour la première fois dans un titre de Resident Evil, la bande-son a bénéficié d'enregistrements orchestraux en direct grâce au Hollywood Studio Symphony Orchestra. La chanson thème «Pray» a été interprétée par la chanteuse Oulimata Niang.
